An intake form master item is a reusable form definition for collecting structured information. You can start with a shared starter template or build a form from scratch.
Open your dashboard and choose Master Documents under Action Pages.
Select Intake Forms to open the form catalog.
To start from an example, choose Browse Marketplace and open Intake forms. Select Preview to inspect the questions, then Install form to add an independent published copy to your workspace. To build your own, select Create intake form from your form library.
Enter a form title and description. In Add field, choose a field type and select Add field.
For each field, set its label, required setting, and any help text. For single-choice and multiple-choice fields, add the choices that should be available. Use the field controls to change the order or remove a field.
Select Save draft version to save an unpublished version, or Save and publish version when the definition is ready to publish. Each save creates a new version; earlier versions remain available in version history.
Choose Preview saved version to inspect the saved definition. Preview uses the latest saved version unless you choose a specific version from version history.
Your intake form library lists forms you created or installed in the current workspace, not other users' private forms. Use Active, Archived, and All forms to filter the list, or search by title and description. Each row shows publication status, field count, and the published version. Use Edit form or Preview directly; open More actions to duplicate, archive, or activate a form.
Marketplace lists only admin-created starters explicitly made available for installation. Publishing your own form makes it usable in your requests, not public in Marketplace. Installed copies remain private to you in your current workspace. The same private-copy rule applies to installed request templates.
Archived forms remain available to existing request records and can be reactivated when your plan allows. Free workspaces can have one active intake form; Pro and Business workspaces have no active-form limit. Installing or duplicating a form uses active-form capacity. Marketplace starters remain unchanged when you edit an installed copy.
Active forms with a published version can be added to client requests alongside documents. Publishing a form does not send it to a client: create a document request, select the form, and send the request. Each request item keeps the published version selected when the item was created, including when you save a request preview. Later edits to the master form do not change that request.
Do not collect full Social Security numbers in intake forms. Starter forms request only the last four digits where applicable. Do not use the hidden clinic demonstration form for real patient information.
