How to Review Intake Items and Inspect Their Audit History

Use form-specific review actions, return answers for correction, and inspect who changed an intake submission and when.

An item marked Form collects structured answers rather than uploaded files. Use the form's review page, not the document Actions dropdown, to manage its submission.

Open a submission

  1. Open the request and find the Form row. Not submitted yet means there is no current submission available for review; a client may still be working on a draft.

  2. When the client submits, select Review form, or use View form in Files / Forms to open an available submission.

  3. Check the client, form version, submission time, status, and submitted answers.

Choose an item action

  • Edit answer: Correct a submitted or approved answer while the parent request is active. Save the change. The original and replacement values, actor, and time are retained. Editing an approved answer does not itself remove approval.

  • Request changes: For a submitted form, enter an overall review comment and select Request changes. The client sees a Changes requested pill and feedback in the secure portal, can update the answers, and must submit again. Feedback is not included in the notification email.

  • Approve form: Accept a submitted form. It now counts as fulfilled; approving the last outstanding form may complete the parent request.

  • Reopen for client: When available and the parent request is active, return the form to an editable draft. Reopening clears its current approval and submission metadata but keeps previous values in audit history. It does not reactivate a completed or cancelled request.

  • Print: Print the review page using the browser. This is not a generated PDF-download feature.

Inspect who changed what

  1. On the review page, find Audit history. Each change shows its action, staff or client identity, timestamp, and status transition.

  2. Expand Answer changes to compare the before and after values. Choice answers can be shown as option identifiers rather than labels.

  3. Read Review history for earlier correction comments and whether they were resolved by resubmission.

  4. Use the request's Activity tab to see staff visits and workflow actions. Its View form audit links open the detailed history.

Audit snapshots are encrypted and visible only to authorized staff, not in the client portal. Tracking begins when audit functionality is enabled: earlier overwritten client answers cannot be reconstructed, although existing comments and activity remain. Saving identical draft answers does not add another change entry.

Workspace owners can access workspace requests; other members are restricted to their own requests. If an action is unavailable, check your access, the form status, and whether the parent request is still active.
