How to Send and Review Intake Forms

Send an intake form to a client, follow their submission, request corrections, and approve the completed form.

Use an intake form to collect structured answers through the same secure request link your clients use for documents. A request can include only forms, only documents, or both.

1. Prepare a published form

  1. Open Master Documents under Action Pages, then select Intake Forms.

  2. Choose Create intake form to build from scratch, or select Browse Marketplace, open Intake forms, and select Install form on a starter. You can preview its questions first. Enter or customize the title, instructions, and fields you need.

  3. Set required fields and choice options, then select Save and publish version. A draft-only form is not available for client requests.

  4. Use Preview saved version to check the saved questions before adding the form to a request.

2. Add the form to a request and send it

  1. Open Requests and select New Document Request.

  2. Enter the client's email address. Choose your published intake form from the item picker and select Add. Add any document items the client should also provide, or use a saved request template containing those items.

  3. Review the recipient, selected items, and instructions. The same intake form can appear only once in a request.

  4. Select Send request to email the client the secure request link. Saving a preview does not open the request for client access; send the saved request when it is ready.

The form version is fixed when the request item is created, including in a saved preview. Publishing new questions later does not change an existing request or a saved preview that you send later.

3. The client completes and submits the form

  1. The client opens the secure email link and verifies their email using the one-time verification code. If the request includes an acknowledgement, they complete that step when prompted.

  2. Under Forms to complete, the client selects Start or Continue. Any requested files appear separately under Documents to upload.

  3. The client enters answers and waits for Draft saved before leaving the page. Draft answers can be continued later through the same request link after verification.

  4. The client selects Review answers, checks the information, then selects Submit form. Required answers must be completed before submission.

  5. The form becomes read-only while awaiting review. The client can select Back to request to continue with other forms or document uploads.

The request creator receives a submission notification. Form answers are not included in the notification email.

4. Review the submitted answers

  1. Open the request from your Requests list.

  2. Find the item marked Form and select Review form after the client submits it.

  3. Read the answers and review history. If a correction is needed, use the answer's edit control and save the change. Staff edits are recorded in the answer history.

  4. Use Print to print the review page with your browser. A generated PDF download is not currently available.

5. Request corrections when needed

  1. On the submitted form's review page, enter an overall comment explaining what the client should change. Add comments beside individual fields where useful.

  2. Select Request changes. The form is returned to the client for updates, and the client receives an email with the secure request link.

  3. The client opens and verifies the request, selects Update answers, reads the feedback, and corrects the answers.

  4. The client reviews and submits the form again. Open Review form to check the updated submission.

Review comments and answers stay in the secure portal; they are not included in the changes-requested email. Earlier feedback remains in the review history after resubmission.

6. Approve and finish the request

  1. When the submitted answers are acceptable, select Approve form. The form item is now fulfilled.

  2. Repeat for any other forms. Submission alone does not fulfill a form; it must be approved.

  3. The request can complete when every form is approved and every requested document has a clean uploaded file. Approving a form does not complete a mixed request while files are still outstanding.

If a submitted or approved form needs to be reopened while the request is still active, use Reopen on its review page. This returns it to an editable draft. Reopen does not reactivate a completed or cancelled request.

If you cannot continue

  • Form missing from the picker: Check that it belongs to the current workspace, is active, and has a published version.

  • Client link unavailable: Confirm the request was sent successfully and is not only a saved preview, cancelled, or completed.

  • Answers changed in another tab: Reload the form and check the latest saved answers before continuing.

  • Review access unavailable: Workspace owners can review workspace requests; other members are restricted to their own requests.

Do not enter passwords, verification codes, or full Social Security numbers in form answers. Upload requested files as separate document items, not as form answers.
